Acknowledgement of Risk and Release
I understand that volunteering, visiting, or participating at Open
Heart Farms may involve contact with horses, equines, farm animals,
equipment, outdoor areas, uneven ground, weather, insects, allergens,
and other conditions that may create risk of injury, illness, property
damage, or death.
I understand that animals can be unpredictable, even when handled with
care. I agree to follow all posted rules, staff instructions, animal
boundaries, safety directions, and program requirements while on the
property or participating in Open Heart Farms activities.
To the fullest extent allowed by Florida law, I voluntarily assume the
risks connected with my participation and release Open Heart Farms,
its directors, officers, staff, volunteers, agents, property owners,
and partners from claims arising from ordinary negligence connected
with my participation.
Medical, Media, and Minor Participant Terms
If emergency medical care is needed and I cannot be reached, I
authorize Open Heart Farms staff or volunteers to seek appropriate
emergency assistance. I understand I am responsible for medical costs
connected with my participation.
I give Open Heart Farms permission to use photos or video from farm
activities for program, volunteer, fundraising, and outreach purposes,
unless I provide written notice that I do not consent.
If the participant is under 18, a parent or legal guardian must sign
this waiver and accepts these terms on behalf of the minor participant.
